Print-Out Order Form

Answer the questions, fill in the blanks, mark your choices, calculate the cost, pack up any photographs you want included, and mail it all with a check or money order to:Mikie Metric Productions, P.O. Box 3216, Williamsport, PA 17701. Only send the sections of this form that pertain to the product(s) you are ordering. If you are ordering a Greeting Card, we do not need the Calendar form. With every order, send the Personal Data form.  Note: We will send you a proof of the work-in-progress for your approval before finalizing the job.

Notice to customers: Mikie Metric Productions is a photo editing, enhancing and manipulating business, not a publisher or printing shop.  We design, set up and lay out the pictures for calendars, book marks, post cards, etc.  and then provide you with one or a very few samples of the end product as well as a CD containing the set-up which you will then send or take to a professional print shop or calendar company, where it will be fed into their computers for actual production.  We also do enhancement and restoration of old or damaged photographs.

CALENDARS: Let us know what picture you want used for each month and for the cover page.  We can use one of our scenes or scenes that you send to us.  We will smoothly blend the photos of up to 25 people into the 12 scenes chosen for the months. Send us the photos of those people. Tell us clearly on a post-it or by writing lightly on the back of the photo which person we should use if there are others in the photo. Also on the post-it or on the back of the photo write the name of the month in which you want the picture inserted. These photos will not be harmed in any way so feel free to send one-of-a-kind pictures. They will be returned to you with your paid-for order.  We will print out one complete set of pictures (cover plus 12 months) on good-quality paper and send that set along with the printing set-up on a CD.  You will then take or send the CD to a printing company or calendar maker for actual production of the calendar, including date pages with personal names or special occasions printed on them.

GREETING CARDS: Make it easy on both of us.  Describe as completely as possible what type of card you want, for what purpose, and what you are providing for us to work with.  We can use the photograph of a person alone or blended into a scene that you send to us.  We could also use one of our stock scenes as the background.  All of our cards are 8 1/2 inches by 5 1/2 inches in either portrait (vertical) or landscape (horizontal) positions.  Cards are printed on heavy card stock paper and come with envelopes.  If you only want one or two cards, we will print them out for you.  If you want more than that, we will put the set-up on a CD which you can take to a printing shop for actual printing.  If you opt for the CD, we will still send you one card printed out as a sample.

Picture or Scene list for Calendar:

Post Cards: Choose a scene, from our Photo List or from your own collection. Pick out a good photo of the person who is to become part of the scene. Send us your photos along with the order form below. We will send you a proof of the job before finalizing it. If you want more than 4 postcards, we will put the set-up on a CD which you can than take to a printing shop for actual printing.  We will still send you a sample card printed out so you can compare it to the printer's version.

Bookmarks: As with the Post Cards, choose a scene , keeping in mind that the bookmark will only show a slice of that scene. If you prefer, you can just choose a type of scene - general sky, sunset, rocks, etc. Send a photograph of the person (or critter) who is to appear on the bookmark. Tell us what wording or name you want us to print with the picture. We will send a proof for your approval before completing the project.
